Completion rate is the only intake metric that matters. Six changes that consistently move it.
Length is not the problem
Patients will answer forty questions if each one feels like it matters and progress is visible. They will abandon twelve questions rendered as an undifferentiated wall.
- One question group per screen with a real progress indicator
- Prefill everything the practice already knows
- Save on every keystroke so a dropped call costs nothing
- Native pickers, never a custom date control
- Plain language, reading level checked
- A clear statement of who sees the answers
